Liability Waiver
I, the undersigned, in consideration of my child’s participation in the dance classes, events, and activities conducted by “Dance With Safa”, hereby agree to the following:

Acknowledgement of Risk: I recognize that dance is a physical activity that involves risk of injury. I acknowledge that despite the safety precautions taken by “Dance With Safa”, accidents and injuries may occur.

Release of Liability: I hereby release “Dance With Safa”, its instructors, employees, volunteers, and agents from all claims, liabilities, or demands for personal injury, death, or property damage occurring during or resulting from my child’s participation in these activities.

Medical Treatment Consent: In the event of an injury to my child while participating in these activities, I consent to medical, hospital, or dental treatment that may be deemed necessary. I accept responsibility for all associated expenses.
Social Media Consent
I understand that “Dance With Safa” may use photos, videos, and other digital media of its classes for promotional, educational, or informational purposes on its website, social media platforms, and other publications.

Consent to Use Image: I hereby grant “Dance With Safa” permission to use images and videos of my child, captured during their participation in dance classes and related activities, in its promotional materials.

No Compensation: I understand that the use of these images and videos is without compensation or additional consent and remains the property of “Dance With Safa”.

Opt-Out Option: If I prefer my child not to be photographed or recorded, I will notify “Dance With Safa” in writing.
